Ordering from Managed Rx Plans is as easy as 1, 2, 3:

1. Ask your doctor to prescribe the needed medication and refills. Have your doctor call in your prescription or you can send it to us via fax, mail or email (controlled medications have special requirements ).

2. Fill out the patient profile below (assignment of benefits). We will contact you, and send you any other forms for additional information. Once you have received these, send them back to us via email, fax, or mail.

3. Sit back and relax! Your prescriptions will be mailed express to your designated shipping address along with re-ordering instructions and future prescriptions and/or refills.
